Today, I want to talk about forehead lift surgery, also known as brow lift surgery.

While forehead lifts are often used as an anti-aging procedure to address drooping eyelids, if you are considering this surgery, it’s important to have a clear understanding of its outcomes.

In reality, the primary goal of a forehead lift is quite simple: to lift the eyebrows.

Let’s try something together: place your hand on your eyebrows and lift them while looking in the mirror. What do you see? You might notice some secondary changes that occur as the eyebrows are lifted.

First, lifting the eyebrows creates more space between the eyes and the brows, which can make your expression look more refreshed. For those with heavy eyelids, you’ll notice that the lifted eyebrows smooth out the upper eyelid area, making your eyes appear more relaxed. If you typically feel that you have "sleepy eyes," lifting the eyebrows can open up your eyes and make them appear more alert. Additionally, if you had double eyelids that were obscured by sagging skin, you might see those eyelid creases become more prominent again.

Try blinking your eyes while holding your brows up. Do you notice that it's easier to open and close your eyes? If you had sagging eyelids before, you might find that this also improves the appearance of wrinkles around the eyes, and the horizontal lines between your brows might become less noticeable.

If you find yourself nodding along with what I’m saying, you might want to consider a forehead lift surgery!

The effects of a forehead lift are fairly straightforward, and it’s worth considering this procedure if you are looking to achieve these specific outcomes.

I’m bringing this up because I see patients of various ages, including many in their 20s. Sometimes, I recommend a forehead lift to younger patients, and they often wonder, "Isn’t this surgery meant for older people?" I want to clear up this misconception.

Forehead lift surgery typically involves making small incisions about 2 cm behind the hairline, usually four or five incisions. An endoscope is then used to separate and lift the entire forehead skin before it is fixed into place. There are two main methods for securing the lifted forehead: using Endotine devices or bone tunneling with sutures. A key point to note is that either method requires the skull’s growth to be complete, which generally means that anyone over the age of 18 is eligible for this surgery.

Some people worry that a forehead lift is a major surgery, but from a surgeon’s perspective, although the area of operation is large, the procedure is not particularly difficult or risky. In fact, if you compare it to a standard double eyelid surgery, the latter can actually be more challenging.

I also often get questions like, "If I have a forehead lift at a young age, will I need another one as I get older?" While it’s true that aging will eventually cause some sagging again, undergoing a repeat forehead lift surgery is generally not an issue. This is because the endoscopic forehead lift only involves lifting and re-fixing the forehead skin, without significant disruption to underlying structures. In my experience, repeat surgeries are usually quite manageable.

While I can’t say for certain how many times this surgery can be repeated, I believe it’s possible to undergo the procedure up to three times without major issues.

After a forehead lift, there is usually sufficient space between the eyes and the brows, so a subsequent surgery might only require a simpler procedure, like revising the double eyelid line, to achieve your desired results.

So, even if you’re young and recommended a forehead lift, don’t be too alarmed—it's worth considering.
